Internship Forms​​

Submit the internship forms to the department at least 5 working days in advance.
Fill in the forms on a computer!!! Print out the forms so that they do not overflow to a second page!!!
You can access the forms by serching the number of the relevant form on the YTU Quality Page.

At the Beginning of the Internship:
 
1. The company which the internship will be held is found by the student, in accordance with the coverages of Internship-1 (Metallurgical Production Processes Internship) and Internship-2 (Shaping Processes and Administration Internship). Please refere to the Internship Regulations for detailed information about internships and their coverage.
2. The name of the internship company is checked in the Internship-1 (Metallurgical Production Processes Internship) and Internship-2 (Shaping Processes and Management Internship) lists. If the mentioned company is in the lists, forms are prepared without receiving a confirmation mail. However, if the company where the internship will be conducted has multiple campuses, information regarding where the internship will take place should be provided. If the name of the company is not in the relevant lists, approval is obtained from the members of the Internship Committee (Lecturer Dr. Yahya BAYRAK and Res. Asst. Zekeriya Yaşar CÖMERT) via e-mail from the address metstaj@gmail.com, as to whether the company is suitable for internship or not.
3. When coming for internship approval, it is necessary for the relevant sections of the 1847 form pertaining to the internship to be fully completed and the processes to be observed marked. Otherwise, internship approval will not be granted.
4. If two internships are to be done in one internship period, internship approval must be obtained by e-mail.
5. Forms 1, 2, and 3 are filled and approved by the company.
6. Forms 1, 2, 3, 4, 5, and 6 are filled and along with a photocopy of your ID, a photo, printout of the internship approval e-mail, and internship calendar (the days of internship must be circled on the calendar), delivered to the Department Secretariat.
7. Form no. 2 is signed by the Internship Committee Chair or Department Vice Presidents.
8. The forms are delivered to the Dean's floor, office no. A-310.They are taken back from the Department Secretariat in 2-5 days as approved.
 
NOTE: In the FR-1847- Form, it is sufficient for the student to arrange the part related to their type of internship (Page 1 for Internship-1 (Metallurgical Production Processes Internship) and Pages 2 and 3 for Internship-2 (Shaping Processes and Management Internship)).
 
At the End of the Internship:
 
1. Forms 1, 5 and 6 (in a sealed envelope) are submitted to the Department Secretariat within 1 month at the latest.
 
2. The practical training diary (internship notebook) is filled in electronically using the "Practical Training Diary" format below. Form no. 1 is scanned and added to the appendix section at the end of the diary. The electronically filled diary is printed out, then all pages are signed with a wet signature and stamped by the company representative. The signed and stamped diary is scanned and uploaded to the relevant web page (https://forms.gle/FLSBH9qAAV4V1qcVA) within 1 month at the latest after the internship is over.

3. The practical training diary is evaluated by the responsible research assistant within 1 month. If the diary is insufficient or a correction is requested, the student makes the necessary corrections within 15 days and reloads the notebook on the relevant web page.
 
4. Internship evaluation result is submitted into OBS within 1 month.
